Fill the activity details before you share it
Open the template in Fill PDF form, complete the top section — the organization, the activity, the dates and times, where you are going and how much it costs — and download it. That copy is what you send to families. Leave the participant and guardian sections empty.
How families complete it
Parents can open the PDF on a phone or computer, type the child's details, tick the permission box and sign with FormFlow's Sign PDF tool or any PDF app. If you want the signature and date spots clearly flagged, run the filled copy through Signing packet before sending it out.
Collecting the replies
When the signed forms come back by email, drop them all into Extract form data to get one spreadsheet of names, health notes and emergency phone numbers for the trip leader. Keep the forms themselves somewhere secure; they contain health information.
Tips
- Check your school district or organization's own policy; some require their official form.
- State the cost and what it covers so families do not have to ask.
- Bring a printed copy of the emergency contact list on the trip.
